The first of the two smaller back teeth (premolars) on the upper right, just behind the canine.
- In front: #6 upper right canine
- Behind: #4 upper right second premolar
- Same tooth on the other side: #12 upper left first premolar
- Bites against: #28 lower right first premolar
It replaces the upper right baby first molar (B), which falls out at 9–11 years.
Usually two roots (1–3) and usually two canals (1–3).

This tooth is #5 in the US and 14 in the international (FDI) system, where the 1 means the upper right quarter and the 4 is its place counting from the front.
There's no international tooth 5, because those numbers always have two digits.
And in the US, #14 is the upper left first molar.
